In the Supreme Court  of Suffolk County I, Sylvester Arasimowicz , aged 34 years,
occupation  carpenter , do declare on oath that my personal
description is: Color   white  , complexion   ruddy  , height   feet   inches,
weight  165  pounds, color of hair  brown , color of eyes   brown  ,
 other visible distinctive marks   none  .

I was born in                                 Poland, Russia                      ,
on the   23   day of   February  , anno Domini 1 886 ; I now reside
at      Fishers Island   ,      NY        .

I emigrated to the United States of America from       Hamburg, Germany     
on the vessel                       "Moltke"                         ; my last
foreign residence was         Poland, Russia        ; I am ___ married; the name
of my wife is         Annie         ; she was born at          Russia       
and now resided at          Fishers Island .        

It is my bona fide intention to renounce forever all allegiance and fidelity to any
foreign prince, potentate, state, or sovereignty, and particularly to   The Republic of Poland
 or Russia or any independent State within the boundary of the former Russian Empire
 
 of whom I am now a subject; 

I arrived at the port of            New York        , in the
 State of        New York       , on or about the      13      day
of        Aug(ust)       , anno Domini 1   905   ; I am not an anarchist; I am not a
polygamist nor a believer in the practice of polygamy; and it is my intention in good faith
to become a citizen of the United States of America and to permanently reside therein:
