"En el pueblo de Yauco, Provincia de Puerto Rico, a las once de la mañana del día diez y siete de Mayo de mil ochocientos noventa y siete, ante Don Pablo Riera Coral, Juez Municipal y Don Antonio Fernández y Colón, Secretario, compareció Francisco Caraballo y González, natural y domiciliado en este término, vecino, de oficio labrador y mayor de edad, y con objeto de que se inscriba en este Registro Civil el nacimiento de un varón que presentó como padre del mismo declara: que dicho varón nació en la casa del declarante hoy veintiún del corriente mes a las siete de la mañana, al barrio de Carmelo de este término, que es hijo legítimo del compareciente y de su mujer Margarita Pacheco, jornaleros, natural y domiciliada en este término, mayor de edad, y lo presentó por primer nombre el de Francisco y segundo el de Pacheco. Fueron testigos presenciales de este acto Don José Quiñones y Don Prudencio Perales, mayores de edad, vecinos de este término, a quienes instruí de las penas en que incurren los que declaran falsamente. Leída esta acta íntegramente al compareciente y testigos, la ratificaron y firman.

American English Translation:

"In the town of Yauco, Province of Puerto Rico, at eleven o'clock in the morning of the seventeenth day of May, one thousand eight hundred and ninety-seven, before Don Pablo Riera Coral, Municipal Judge, and Don Antonio Fernández y Colón, Secretary, appeared Francisco Caraballo y González, native and resident of this district, neighbor, occupation farmer and of legal age, and for the purpose of registering in this Civil Registry the birth of a male child whom he presented as his son, declares: that said male child was born in the house of the declarant today, the twenty-first of the current month at seven o'clock in the morning, in the Carmelo neighborhood of this district, that he is the legitimate son of the appearing party and of his wife Margarita Pacheco, laborers, native and resident of this district, of legal age, and he presented him with the first name Francisco and the second name Pacheco. Present as witnesses to this act were Don José Quiñones and Don Prudencio Perales, of legal age, residents of this district, whom I informed of the penalties incurred by those who declare falsely. This act having been read in its entirety to the appearing party and witnesses, they ratified and signed it.
